What is an SIR and what does it do? (husband is AD)

I have a question and i’m very confused about what’s going on rn. I had a suicide attempt 2 days ago, and my husbands command submitted a SIR (significant incident report? i think?) and everyone with my husband at the ftx he’s in had their phones taken away yesterday morning. they said they were going to look into what they can do? what does this mean? what should i expect? idk i’m just really confused and i don’t even know what an incident report is?
